Transition to Home

Returning home from hospital or rehabilitation is a difficult and often unsettling time. We know the first 24 to 48 hours at home are the most critical and this is when our support can greatly reduce the risk of re-hospitalization, help with your recovery and strengthen your self-sufficiency.

You may have a visiting nurse scheduled to come by or your family will be around, but will they be there when you really need the help?  We can prepare for your arrival home, shop for fresh food, prepare small meals and assist with transfers from one room to another.  It will be important to take care of yourself those first few days, rest to regain your strength and not over do it.  You’ll be glad to be home.  Let us help you stay there safely.
